Health Center: Policies
The University Health Center is committed to providing the best possible health care to our students and members of the community, while respecting their rights to privacy, confidentiality and autonomy. We post our policies regarding care and confidentiality as a service to our clients. Please click on the links below to view the policies we follow in providing care:
Health History Form
All students at the University are required to file a health history form with the health center. Oregon state law requires that each entering full-time student must have two doses of measles vaccine or MMR vaccine (documented by month and year of each dose) on or after the first birthday, with a minimum of 30 days between doses. International students are required to have a tuberculosis skin test (PPD) performed by a United States health care provider no later than two (2) weeks into the semester. School of Nursing students are required to complete the mandatory immunizations section.
Confidentiality Statement and Consent to Treatment
Communication between you and University Health Center Staff are confidential as required (and except as limited) by law. Generally, information may not be disclosed to another person or agency outside of the University Health Center (including parents, teachers or Residence Life staff) without your written consent.
