Web & Administrative Systems: PerfectForms

PerfectForms is online survey, form, report and workflow creation application that WAS supports for the University.  It is easy to use so faculty and staff can create forms and surveys to post on websites or distribute via email.  As form data comes in, it is collected and stored in an IS-supported SQLServer database.  Form/survey owners are able to export their data into a variety of formats or build reports to dynamically view the data online.

PerfectForms are currently being used across campus to gather award nominations, training or product evaluations, questionnaires, event registrations, information requests, and applications; post surveys; move through information workflows; and collect simple online payments.  The College, professional schools, and administrative and student offices currently have active forms.  Please see the list below for a few examples.
