Staff, if you are experiencing difficulty with completing the form, please contact hr@up.edu or leave a voice mail for a return call at: (503) 943-8484. Please do not to come to campus if you have COVID-19 symptoms or believe you have been exposed.

For questions about vaccines, or scheduling an appointment to get care or testing please contact hcc@up.edu. 

The Health and Counseling Center is open weekdays to offer PCR and antigen testing to any student with COVID-19 symptoms. Antigen self-test kits will be available 24 hours a day at UP Campus Safety for any UP community member experiencing symptoms of COVID-19.
