Vaccinations

University of Portland strongly recommends students be fully vaccinated and stay up-to-date on their vaccinations by receiving the most recent recommended boosters.

If you were born on or after January 1, 1957, Oregon law requires you to submit proof that you’re protected against measles. To meet this requirement, you must submit documention from ONE of the following options:

Option 1: Official immunization record including:

  • Two doses of the MMR (measles, mumps, rubella) vaccine
  • Doses given at least 28 days apart
  • First dose given after your 1st birthday

Option 2: Approved proof of immunity:

  • Positive measles titer (lab report showing immunity)
  • Signed medical provider statement confirming a past measles infection (with date)

Option 3: Exemption:

  • Signed medical provider statement verifying a medical exemption
  • Non-medical exemption with Vaccine Education Certificate (completed annually

Note: If you have an exemption and are exposed to measles and have to quarantine, you will likely be placed on a medical leave of absence (MLOA) for the semester.

  1. Login to the Patient Portal. with your UP email and password.
  2. Go to the Medical Clearances section.
  3. To enter vaccination dates
    • Next to 'Measles,' select 'Update.'
    • Click 'Add Immunization' to enter each MMR dose.
    • You must enter dates AND upload documentation to be marked compliant.
    • If you are submitting alternate proof of immunity or exemption, skip to step 4.
  4. To upload documentation (e.g. immunization record, titer result, Vaccine Education Certificate, record of medical exemption)
    • Next to 'Immunization Records,' select 'Update.'
    • Select 'Upload' to attach your documents.
    • Uploaded records MUST show your full name and date of birth. 
  5. To check your compliance status
    • Return to Medical Clearances
    • Status will update to Compliant after review by our team. Please allow up to 5 business days.

A registration hold may be placed on your account for the following semester if this information is not uploaded 1 week prior to Fall or Spring break.
