Health Center: Request Accommodations
The Office for Students with Disabilities (OSWD) model is semi-centralized. Efforts to accommodate students with disabilities are coordinated through the OSWD, but at the same time, much of the implementation is carried out by the students themselves. Students who wish to exercise their right to accommodations (provisions made to allow equal access to University programs) must follow the following procedure:
1. Contact the Coordinator.
2. Complete the Request Form.
3. Provide Documentation.
4. Evaluation, Recommendation and Plan.
5. Implement Your Accommodations.
STEP 1: Meet with the Coordinator.
Students making an accommodation request must contact the Coordinator of the Office for Students with Disabilities, located at the University Health Center, Orrico Hall, (503) 943-7134 for information about the Accommodation process.
STEP 2: Complete the Request Form.
Download the Accommodation Request form, fill it out, and return it to our office.
STEP 3: Documentation
Students must provide documentation of the disability, such as a report from a physician or psychologist. Details of documentation requirements are available from the OSWD.
Please Note: Students should keep the original or a copy of all submitted documentation for their own records.
STEP 4: Evaluation, Recommendation and Plan.
The staff of the University Health Center form a Multi-Disciplinary Team (MDT), which reviews the documentation. The MDT includes members of the UHC staff who work with an inter-disciplinary focus to evaluate documentation of disabilities and to suggest appropriate accommodations. The MDT is comprised of the UHC Director, the Coordinator of the OSWD, a psychologist, and a nurse practitioner. The team verifies the disability and makes recommendations regarding accommodations.
Based on the recommendations from the MDT, the Coordinator will create an accommodation plan, which includes reasonable accommodations, to be reviewed and signed by the student. The plan must be approved by the Dean or the Dean’s designee.
STEP 5: Implement Accommodations
The student should communicate with all appropriate parties in an on-going manner to assure the best realization of the Accommodation Plan.
Before classes start, set up appointments to meet with your professors, preferable during their office hours. Talk about your disability, and present each professor with an Accommodation letter. Then develop a plan for the semester. If you’ve been approved for exam accommodations, bring the Exam Accommodation form with you so your professor can complete their section.
If you’ve been approved for alternative text accommodations, exam accommodations, or note takers, you are strongly encouraged to attend an OSWD Training Session at the beginning of each semester.
