Submit an IBC Application for Work with Biological Agents for new projects and renewals of projects approved three years ago (five years for classroom projects.) A new IBC Application for Work with Biological Agents must be completed every three years (every five years for classroom projects.)

Submit an Amendment and Annual Renewal Form. The Amendment and Annual Renewal Form must be submitted each year for each approved application. This form should only be used for projects that have been approved less than three years ago (less than five years ago for classroom projects.) A new IBC Application for Work with Biological Agents must be completed every three years (every five years for classroom projects.)

Submit a Personnel Form to report students and others who are involved in your research project but are not enrolled in a course for which you have IBC approval. Any additions or changes to students or other personnel involved in the project should be reported to the IBC. You will save this document to your computer, complete and email the completed form to IBC@up.edu.
